The Ottawa District Hockey Association is pleased to announce that as a parent of an ODHA minor player, an accidental death & dismemberment insurance policy is now available at NO COST TO YOU. This policy is being made available through American Income Life Insurance Company. The ODHA is proud to jointly sponsor this NO COST benefit with AIL Canada, a company serving Associations and their families nationwide.

No Cost - $2,000 Accidental Death & Dismemberment Benefit

In Addition, you will also receive:

- Family Eye Care Program
- Quest Travel Plan, Hotel, Cruise Line and Car Rental Discounts (Small activation fee required)
and
Child Safe Kit

If you fail to designate your beneficiary, the benefit will be paid to your estate, which may be subject to probate. This mailing was printed and paid for by AIL Canada, however in order to respect the confidentiality of members, it was labeled and mailed by the Ottawa District Hockey Association.
