No Cost - $2,500 Accidental Death & Dismemberment Benefit To receive the family eye care discount program and the Child Safe kit, members must return the enclosed yellow card or complete the card on-line. The discount card and Child Safe kit are provided to the family at no charge and will be delivered along with your certificate of coverage. |
